2.) A deposit of $100.00 (You're not registered until we have your deposit.  Make checks payable to CUMC and write your child's name in the memo.)

3.) 2010 Breakthru Covenant (Must be notarized.)

4.) 2009/2010 CUMC Student Activity Medical Release Form

Final Balance of $110.00 is due December 3rd.
